Project Description: Project is proposed to provide a replacement facility for the existing Glen Haven Home located at
302 6th Street in Glenwood, Iowa. The existing facility was constructed circa 1964. The existing facility’s configuration
is outmoded, and the facility is at the end of its useful life in terms of physical condition. The replacement facility proposed includes renovation and adaptive re-use of six (6) existing one-story slab-on-grade
buildings, each approximately 6,000 square feet, located on the campus of the State-owned Glenwood Resource
Center in Glenwood, Iowa. The subject buildings will be leased by Glen Haven, from the State of Iowa, and the
proposed renovation work will be considered leasehold improvements. The subject buildings were all built, circa.
1978-80, as residential-style group homes. Five (5) of the renovated “cottages” will be configured to support skilled
nursing care functions. A sixth “cottage” will be configured to support rehabilitation functions. Note: # 136 Indian
Hills Drive, a seventh existing identical building, skilled nursing care, is included in this set of Documents as an add-alternate.

Contractors may obtain drawings and specifications for the work from the office of A & D Technical Supply Co., located at 4320 South 89th St., Omaha, NE 68127, (402) 592-4950 ex. 4, or by order through their Virtual Plan Room at www.adtechsupply.com.
A Refundable Deposit of $100.00 or a Builders Cards will also be accepted in lieu of deposit check to obtain Bidding Documents.
Checks are to be made payable to Glen Haven Home, Inc.
Any bidder upon returning the documents in good condition within 14 days of the public opening of the bids, shall be returned the deposit in full.
If shipping is required, there is a flat rate of $35.00 per set (Please address all checks for shipping to A&D Technical Supply Plan Room).

CD's or Downloads in PDF format of Complete Sets are available purchase only $30.00. All checks for purchases are made payable to A&D Technical Supply Company.
