Each respondent must submit one electronic copy and three paper copies. Electronic copies to
be saved on a USB drive. Responses to Appendix A should be clear and concise. Do not
include additional vendor product literature other than information and images used for
responses to specific questions. Mail or deliver USB drive and three hard copies to the
following: University of Nebraska Medical Center, Procurement Department, Attention: Victoria Halinski
985050 Nebraska Medical Center, Omaha, NE 68198-5050.
Any questions regarding this RFQ should be directed to Toby Samuelson at
tsamuelson@morrisseyengineering.com. Questions and applicable response will be sent to all
companies participating in the RFQ process..
RFQ's are due by 2:00 PM on July 20, 2016.
