Public Notes:
Work consists of the construction of a 16,710 square foot three story Specialty Clinic addition, 2,872 Physical Therapy addition and renovation of the existing PT/CR Department (including lower level mechanical pit for the new Physical Therapy pool) & a 156 Electrical Room addition at Brodstone Memorial Hospital in Superior, Nebraska. Other renovation work in the existing facility includes; Work at the existing Specialty Clinic to be converted into additional exam room, procedure room and doctor's offices for the adjacent clinic. Converting the existing Purchasing/Material Management space into new Gift Shop and Meeting Room. The Project is divided into three construction phases and they are as follows: Phase I - Construction of the New Physical Therapy pool addition with treatment rooms, renovation of the existing Physical Therapy & Cardiac Rehab Department & the construction of a new PT/CR parking lot; Phase II - Construction of the new 3 story Specialty Clinic addition; Phase III - Renovation of the existing Specialty Clinic of the existing facility into Clinic space.

Bid Documents may be obtained from the office of A&D Technical Supply Co., Inc., 1822 N Street, Lincoln, NE 68508, (402) 474-5454 upon receipt of a refundable deposit check in the amount of $100.00 payable to A&D Technical Supply Co., Inc. *A Builder's Card will NOT be accepted in lieu of deposit check.

CD's containing a full set of Bid Documents in PDF format are available upon receipt of a non-refundable fee in the amount of $25.00 payable to A&D Technical Supply Co., Inc.
No partial sets or downloads will be available.

If shipping of Bid Documents is required, there is a non-refundable fee in the amount of $30.00 payable to A&D Technical Supply Co., Inc.
If shipping of CD is required, there is a flat fee in the amount of $9.00 payable to A&D Technical Supply Co., Inc.

Bid Documents must be returned in good, usable condition within 30 calendar days of bid date to obtain a deposit refund.
If bid documents are not returned within the stipulated time frame the deposit will be forfeited.
