AM Central Time on Friday October 18th, 2024 at the office of Maple Park Dental Associates at 805 S Maple Street,
North Platte Nebraska.

This project consists of a new 6,609 square foot two story wood framed structure addition to the South end of the existing
facility. Exterior walls shall be 2x6 with R-21 batt insulation. Roof consists of R-38 batt insulation with wood trusses and
partial relocation of some existing timber trusses with new T&G roof decking. There is a second floor with floor trusses and
wood decking. Finishes are intended to match the existing facility. The floor slab is to be 4” reinforced concrete slab over
15mil vapor barrier on 4” base bed. The foundation consists of grade beams with 2” rigid insulation full depth. The contractor
is required to take great care in relocating the existing timber truss system being modified, relocated, and re-installed in good
condition and matching exact existing dimensions. The contractor shall also take great care in field verifying the exact size,
shape, pitch, energy heal heights, overhangs, etc. at both the timber truss and entire South end of the building as so the new
trusses flush up against the existing to prevent any slight “humps” where the new meets the existing as the overall esthetics are
crucial on this project. The entire South face exterior finishes (stone, siding, eyebrow roof, fascia, soffit, etc. shall be removed
and the surface cleaned to a smooth finish to allow the application of a 2-hour fire paint rated for wood surfaces. This 2-hour
fire paint application shall overlap where the wall meets the slab and shall overlap where the wall meets the roof and then
around the West and East exterior corners 3” minimum for a complete wall coverage for a new 2-hour wall separation. There
will be a new parking lot to the North of the existing facility that will be built once the existing house has been removed. Site
preparation for the parking lot shall be part of this bid package.
