PREPARATION OF BIDS
.1 Bids shall be submitted on unaltered Bid Forms furnished by the Architect-Engineer.
.2 Each Bid shall include the legal name of the Bidder, and shall show whether the Bidder is a
corporation, a partnership, or a sole proprietor, or any other legal entity. A Bid of a corporation
shall give the State of incorporation, and shall have the seal affixed. A Bid of a partnership shall
give the names of all the partners. A Bid of a sole proprietor doing business under a trade name
shall give the name of the sole proprietor and the trade name under which the individual is doing
business.
.3 Fill in all blank spaces for bid prices in ink or typewritten words, and submit one (1) copy. The
Bidder must include all Unit Cost items and all Alternates if shown on the Bid Form. No
segregated or qualified bids will be accepted.
.4 Bidders submitting Bids for Combined Contracts shall enter the names of the Prime
Subcontractors in blanks provided on the Bid Form.
.5 Bidders shall include with the Bids “Bid Form Attachment No. 1 – Public Improvements Project
– OPW53941-PD, ST, SSD – Bid Quantities.” Unit Price and total Cost information shall be
filled-in on the form in the columns provided.
.6 Bids shall be signed by the person or persons legally authorized to bind the Bidder to a contract.
A Bid submitted by an agent shall have a current Power of Attorney attached certifying the
agent's authority to bind the Bidder.

Public Notes:

|
Construction will include public infrastructure such as paving, sanitary sewers, drainage facilities, erosion
control, seeding, and landscaping. Water, gas, electrical and communication facilities will be completed by
utility companies. The Project will include Work along the two school lots and Outlots "A" through "H".
Public infrastructure primarily involves the extension of N 171st Street from Rachel Snowden Parkway to
Military Road and the construction of Woodney Street west from N 168th Street to N 171st Street. The
Project includes two-lane and three lane residential roadway sections, a single-lane roundabout, and
associated utilities

Bid Documents may be obtained from the office of A&D Technical Supply Co., Inc., 4320 S. 89th Street, Omaha, NE 68127, (402) 592-4950, upon depositing the sum of one hundred dollars ($100.00) for each set of Documents. Checks shall be made out to DLR Group Inc. Builders Cards will also be accepted on this project.
CD's and Downloads containing a full set of Bid Documents in PDF format are available upon receipt of a non-refundable fee in the amount of $30.00 payable to A&D Technical Supply Co., Inc.
Only complete sets of Bid Documents may be obtained.
If shipping of Bid Documents is required, there is a non-refundable fee in the amount of $35.00 payable to A&D Technical Supply Co., Inc.
If shipping of CD is required, there is a flat fee in the amount of $9.00 payable to A&D Technical Supply Co., Inc.
Bid Documents MUST be returned, in good condition, within 30 calendar days of Bid Opening to obtain a deposit refund.
